Presenters:
FPD Sign In Sheet This is a fillable PDF to print and use at your FPD event to record FACULTY attendance only. All FPD events must require faculty to sign in and out on this sheet. The presenter must email the completed sign-in sheet to the Senate Coordinator, at Darlene.OKeeffe@avc.edu, the next day after your event. Your attendees will not receive the proper credit if the sign-in sheet is not turned in. Please ask your attendees to print legibly on the sign-in sheet. If the name cannot be read, then that faculty will lose those hours.
Use a different sign-in sheet for students and staff as these should not be turned in. Student/Staff sign-in sheets are for your personal records. Only faculty sign-in sheets should be emailed to the Senate Coordinator.
FPD Change & Cancellation Request Form 25/26 - Presenters use this online form to request changes to OR cancellation of your 25/26 event(s). Please be detailed and specific in the changes needed for your event.
25/26 New Event Request Form - Use only to request new events that will take place during the 25/26 AY.
Attendees:
Be sure to:
- Use the Vision Resource Center (VRC) Events Calendar to sign up for calendared events by clicking the 'Request' button in the event details.
- For physical, in-person events: Sign in using the events' physical sign in sheet when you arrive at the venue. Be sure to write legibly so you get the FPD credit you earned.
- For Zoom sessions: Be sure to sign in using your official AVC Zoom account so that your full name is displayed. Chat is not used for attendance. If your name is not on the Zoom log pulled after the event, we will be unable to record your attendance as there is no documented proof that you indeed attended. If you use a non-AVC Zoom account to sign in, make sure your first and last name is displayed correctly. If it wasn't, email the Senate Coordinator, with the details so she can record your attendance.

* VRC Calendared Trainings = Trainings and events that are hosted by AVC and listed on the Vision Resource Center (VRC) Events Calendar.
** External Trainings = Non-AVC hosted trainings whether listed on the VRC Events Calendar or not, and AVC hosted trainings & events NOT listed on the VRC Events Calendar. For example, conferences, division/department meetings, Keenan Trainings, committee participation, etc. See FPD Handbook for more specifics. Also, AVC hosted events and trainings that ARE listed on the VRC Events Calendar, but the instructions on the list tell faculty to enter as an external event. This includes events such as fairs and festivals, concerts, and open houses, due to the fact that you can come and go at your leisure and your attendance is not required for the period of time set in the VRC events calendar.
